Developing an E&M Chart Review Process

There is a difference between a chart review and a chart audit—but what is it? Does your provider's documentation support their patient encounter? This session will establish a four step process—review charts, educate providers, develop templates and ongoing review—that will help create an impact with your chart reviews. The four steps equip you to concretely communicate findings to providers. As a brief example, you will also learn: the differences between utilization of specialties, how to evaluate documentation compared to what was billed, and be able to differentiate between a Consult vs. Referral.
